Prior to admission, the following must be submitted:

• Completed application form
• Description of home situation
• Relevant medical, psychological, and legal info.
• Signed contract and release forms
• Signed financial agreement
• Photograph

Admission of each applicant will be determined by the Board of Directors based on the information provided and the ability of the program to facilitate treatment of the individual. The progress made by each participant will be monitored by staff and parents, and the date of their discharge may be determined by either party.  If it is determined that the teen is not yet ready to return home upon discharge, COLT will make recommendations for alternative care and treatment.
